The questionnaire (Genealogical Tree Report) represents the structure of the family tree in reference to the refugee. Filling the form is a simple procedure and only demands some attention so that the details are accurate and reported according to the "Fill-in guidelines". The questionnaire is available in printed and electronic form As shown in the picture on the right, the printed questionnaire is available in our offices and in the "Hellenic Cosmos" Cultural Centre. Alternatively, you may ask for it to be posted to you by calling us on the phone number +3010-3422272, or by filling in the special participation form. The same details can be submitted in electronic form 1. Choose the file by right clicking here. 2. Save it on your hard disk. 3. Read the instructions carefully. 4. Fill in the form after you disconnect from the Internet. 5. Save the file.
