DECLARATION OF CONCESSION AND MAINTENANCE OF RIGHTS OF GENEALOGICAL INFORMATION

The undersigned....................................................................................................................... of ..........................................................................................., with the present document I declare expressly and unconditionally to the legal entity that operates under private law, named Foundation of Hellenic World (hereinafter mentioned as FHW) and located in 38 Poulopoulou Street, Athens, with regard to the information that is contained in the attached printed or digital Genealogical Tree Report (hereinafter mentioned as "delivered") included in Annex 1, which is attached and constitutes a continuous and integral part of my present statement, the following:

a) That by granting the delivered to the FHW I am acting freely and on my own initiative, without any kind of pressure or exhortation on the part of FHW to me or to any person in my environment.

b) That any rights relevant to the delivered, belong to me and in any other case, I have secured the agreement of the person/persons to whom those rights belong to, in order to grant the delivered to FHW.

c) That I accept expressly and unconditionally, that FHW is going to keep an archive of the delivered, according to the provisions of Law 2472/1997 on the "Protection of Individuals with regard to the Processing of Personal Data". Furthermore, I expressly and unconditionally accept that the FHW is going to present the delivered to the general public in the following manner:

1. In a relevant exhibition, museum or a cultural event of another kind.

2. By incorporating, using and utilizing it in an author´s work and general audiovisual work, either by appropriately editing it or not (please fill in YES or NO)_____

3. By incorporating and generally using and utilizing it in scientific research programs, educational programs, scientific presentations, either by appropriately processing it or not.

4. By using it to infer and publish overall statistical or other results in all kinds of media (electronic or other).

5. By presenting it to the general public using electronic media, including the Internet (please fill in YES or NO)_________

d) I do not wish to receive any form of payment from the FHW, either now or in the future, for granting the delivered or for the aforementioned uses of it. However, I do maintain the right at any moment in the future, to ask the FHW in writing, to return the delivered to me and remove it from its relevant archives or cease using it in any way. The FHW is obliged to comply without any delay of its own liability, to such a demand of mine. Furthermore, I maintain the right to be informed regularly and in detail, for the use, utilization and the general outcome of the delivered that as abovementioned I granted to the FHW, especially when I submit a relevant written request to the FHW.

The present was signed by the above declarant___________________ in two (2) identical master documents, one of which was given to the declarant and one to the "Foundation of Hellenic World".
